Summarize purpose of the contract, including why it was necessary: Diabetes: MN Medicaid covered the National Diabetes Prevention Program in 2016, however, the pathway to becoming a MHCP/Medicaid Provider and billing process were not clear for community providers, with little payer billing experience. Contractor consulted with the MDH Diabetes Program to clarify billing steps and requirements from DHS and to build knowledge and resources for community providers.
Cardiovascular Health: Contractor also provided technical assistance to selected federally qualified health centers on how to engage non-physician provider team members (community health workers) to support patients more effectively with hypertension and cholesterol management.
