Summarize purpose of the contract, including why it was necessary: To increase access to the National Diabetes Prevention Program (National DPP) for people in MN, the MDH supported Trellis to build a National DPP umbrella hub organization (UHO). The Trellis UHO contracts with MN Health Plans as a Medicaid provider, Medicare supplier and general provider of the National DPP for insured beneficiaries covered by Medicaid, Medicare and/or commercial insurance. Trellis administers the UHO national recognition data reporting and other legal requirements, and manages billing and subcontracts with community and health care providers to serve people with prediabetes in MN.

If this was a single source contract, explain why the agency determined there was only a single source for the services (if applicable) Trellis is an Area Agency on Aging that created and manages Juniper, a unique statewide network of evidence-based wellness programs and a community care hub which screens and refers clients to social supports, helping Minnesotans improve their health and well-being and age well in their communities. Trellis was the only statewide organization prepared to invest time and resources in the intensive process of becoming a National DPP UHO, and to become a National DPP Medicaid provider and Medicare supplier, with key infrastructure (in-house data tracking, reporting and billing systems and health plan provider contracts).
